Affiliate Time Release Donation Form
Per the CSLEA contract, Unit 7 employees may donate leave credits to the affiliate organization's release time bank for purposes of affiliate business. Many of the affiliates need your donation in order to conduct board meetings, prepare collective bargaining language, and other business that affects your benefits and working conditions. So please consider a donation to your affiliate organization. You can access the new donation form on-line by clicking on the PDF file below.
Please mail the original to CSLEA in Sacramento and make a copy for your records. Please note that this donation form does not replace any form required by your agency.
